You may access the following forms, in case there is a difficulty with our online forms. Please print and fill out the desired form, then bring it to your appointment.

Affordable Payment Plans
Orthodontic care is a smart investment in your health and confidence, and we’re here to make it accessible. We offer interest-free, in-house payment plans that allow you to spread the cost of treatment over time, making it easier to manage financially.
We also accept credit card payments and automatic bank drafts for added convenience. Our team will walk you through all available options to create a payment plan that works for you.
Using FSA and HSA for Orthodontic Treatment
You can use Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA) to cover orthodontic expenses with pre-tax dollars. Here’s how it works:
- FSA: You can contribute up to $3,300 per year (double that amount with a spouse, 2025).
- HSA: Individuals can contribute up to $4,300, or $8,550 for families (2025).
If your treatment extends over multiple calendar years, you may be able to use funds from more than one plan year to maximize your benefits. Be sure to check with your plan administrator to confirm your eligibility.
Insurance Benefits
If your insurance plan includes orthodontic coverage, we’ll help you make the most of it. Many plans offer a lifetime orthodontic benefit separate from regular dental coverage.

- Verify your insurance benefits
- Review your coverage with you
- File all claims on your behalf once treatment begins
Please note: Insurance policies vary, and changes in coverage during treatment may affect your benefits. If that happens, the financially responsible party will be accountable for any remaining balance.
